Several programs now sell fractional-dose plans at lower prices. They do not map onto the 2.5–15 mg ladder every comparison uses.
Microdose GLP-1 protocols — fractional weekly doses below the labelled starting dose — are now offered by several tracked programs at prices well under their standard plans. NexLife lists tirzepatide microdosing from $147 a month on a twelve-month plan against $186 for its standard injection.
The clinical evidence base is not the same. The SURMOUNT programme studied 5, 10 and 15 mg maintenance arms. Fractional dosing is a practice pattern, not a studied regimen, and outcomes at those doses are not established by the trials that produced the headline weight-loss figures.
That is why our ranking tables use the standard weekly injection: comparing a microdose price against a full-dose price would make a programme look cheaper for selling something different.
Microdosing may be entirely reasonable for a patient who tolerates standard doses poorly. It is a conversation with a prescriber, not a way to make a price comparison look better.
